North Bay Hydro Distribution
Limited (NBHDL) is pleased to be your provider of electricity distribution
services.To provide you with reliable
source of electricity, NBHDL needs to collect and use certain personal
information about you.As of January 1,
2004 the federal legislation protecting your privacy requires that NBHDL obtain
your writing consent to collect, use and disclose your personal information for
identified purposes.We invite you to read
this notice carefully to understand our policies and practices with respect to
personal information.
The nature of personal
information we collect may include:
Information we receive from you such as your name, address, contact information
and general financial , credit and reference information;
Facts about your historical and current consumption of power;
Information about your transactions with us, such as meter number,
account number, account balances, payment history, and account activity;
Identifying information, such as driver’s license.
North Bay Hydro Distribution
Limited used the information we collect for the following purposes:
To provide you with continuous electric service and to bill you for
that service;
To assist us in the collection of accounts;
To respond to your inquiries about energy use and billing;
To prevent fraud with respect to both you and our company;
To meet legal and regulatory requirements.
Because of the structure of
the electricity sector in Ontario,
it may be necessary to share your billing and consumption information with
third party billing and settlement agencies.For example, your billing and consumption information may be provided to
a retailer with whom you have chosen to enter into a separate contact.Your information may also be disclosed or
shared with other agencies or organizations as required by law or regulation.
We have developed and
implemented a Corporate Privacy Policy for maintaining the confidentiality and
security of you personal information.At
any time, you have the right to request access to your personal information
that we have collected and to request amendments to person information about
you to ensure its accuracy and completeness.To make a request for access to personal information we may have
collected, disclosed or used about you or to request that your personal
information be amended, please contact our Privacy Officer.
I have read and understand
this statement of NBHDL regarding the collection, use and disclosure of my
personal information, and I hereby consent to have NBHDL collect and use my
personal information for the purposes stated above.
CONSENT FORM
The customer agrees to abide
by the Distributor’s Rules, Regulations and Conditions of Service, in effect
and as amended from time to time.
The customer further agrees
to:
1.Pay the Distributor for any electrical energy purchased through the
Distributor as specified by the Customer or their agent.The customer or their agent agree: to pay all
the related service and distribution charges and any other applicable charges
at the location covered by this agreement from the date herein until such time
as the customer no longer requires the service.
2.Commence payment upon receipt or by the date specified by the Distributor.A late charge will by administered on all
accounts after this date.
3.Pay all accounts monthly according to the class rating of the service
for such energy and service in accordance with the applicable Distributor rate
schedule.
Security
Deposits: A security deposit is
required on all accounts.
It may be waived under the following circumstance(s);
·Five years for a general service
Consumer in a <50Kw demand rate class
·Seven years for all other general
service Consumers
·You have been a general service
consumer with another gas or electric utility and can provide a credit
reference letter stating your good payment history.General Service consumers require 5 years
good payment history.
Your security deposit earns interest.The interest is credited to your account at
least every 12 months or on return of your security deposit, whichever comes
first.
I/We hereby apply to North
Bay Hydro Distribution Limited (NBHDL) for electric service at the service
address shown above.I/We agree to pay
for such service(s) as bills are rendered and comply with the conditions for
service may be terminated and not restored until payment is made in full
including a charge for collections, reconnection and a deposit (if not already
collected).I/We will also inform NBHDL
at least one week in advance of when I/We plan to vacate the above premises.
The undersigned consents to
obtaining of credit and/or personal information as may be required at any time
in connection with the credit herby applied, any renewal or extension thereof
and to the disclosure of any credit information concerning the undersigned to
any credit reporting agency or to any person with whom the undersigned has or
purposes to have financial relations.
The utility will make every
effort to ensure bills are accurate; however, billing errors can occur.The utility reserves the right to collect
under billed amounts at any time.
I am of legal age and have
the authority to bind this contract.
This information is collected in accordance with the Ontario Energy Board
Act, 1998 and the Personal Information Protection and Electronic Documents Act
(PIPEDA) and will be used for the purposes of billing and collecting.Deposit Refund policy available upon request.
A security deposit is required on all accounts. It may be waived under the following circumstance(s);
You have been a North Bay Hydro residential customer within the last two years and have 12 months of good payment history* General Service customer require 5 years good payment.
You have been a residential customer with another gas or electric utility, within the last two years, and can provide a credit reference letter stating your good payment history* General Service customer require 5 years good payment history.
Your security deposit earns interest. The interest is credited to your account at least every 12 months or on return of your security deposit, whichever comes first.
Your deposit is refunded back to your hydro account after one full year of good payment* history
IMPORTANT THINGS YOU SHOULD KNOW WHEN USING THE CALCULATOR:
(1) These calculations are related to the electricity portion on your bill only. The estimated discount is based on the difference between the rates set and provided by the Ontario Energy Board for both FHP adjusted and non-adjusted TOU pricing.
(2) These calculations are an estimate for informational purposes only. The calculated figures do not include line-loss adjustments to account for electricity lost during its transmission and delivery between the generators and consumers.
(3) The calculator is not designed to estimate total bill savings and does not include the 8% rebate that has been shown separately on your bill since January 1, 2017. This calculator provides you with an estimate of additional savings related to new TOU pricing only.
